This multidisciplinary guideline aims to support healthcare providers and people with cancer in making decisions about the most appropriate aftercare and rehabilitation. This guideline is intended for use in Belgian (primary) healthcare by all healthcare providers involved in the care of people with cancer after discharge from the hospital and in their ongoing follow-up, in collaboration with the respective organ specialists and oncology departments. This multidisciplinary guideline consists of two parts. The first part addresses the clinical questions related to the care needed to treat and support significant physical and psychological complaints during the rehabilitation period of people with cancer. The second part of the guideline focuses on the collaboration between different primary care providers and their cooperation with secondary care for people with cancer in the same phase of care (aftercare and rehabilitation). During this peer review (in French and Dutch), the guideline will be presented, and we will discuss how it can be applied in our daily practice.
